Registered Nurse (RN) | Case Manager Location: Green Bay, WI Agency: Cynet Health Pay: $3,006 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/9/2026 About the Position Job Title: Hospice Case Management RN Profession: Nursing Specialty: Operations - Home-Hospice Care Duration: 13 weeks Shift: 8-hour Day Shift Hours per Shift: 40 Experience: 2 years of RN experience License: State RN License Certifications: BLS Must-Have: - IV skills - Chest tube maintenance/pleurex drain - Catheter insertion - Tube feedings - Triage skills - Wound care - Wound vac - Comfort dealing with PIV, midline, and central line - Competency with end-of-life care, including pain and symptom management (pharmaceutical and non-pharmaceutical) - Experience with opioids and equianalgesic table - Documentation on EMR platform - Independent critical thinker/problem solver who knows when to reach out for help - Flexibility Description: This is a hospice case management position. Staff will primarily see patients in the community setting, which includes their homes, homes of family members, assisted living facilities, group homes, and nursing homes. Hospice patients are typically at the end of life, with a life expectancy of 6 months or less. Common diagnoses include 40% cancer and 60% other medical issues. Patient ratios in the remote/field setting involve a caseload of 13-15 visits per day, averaging 5-6 visits for an 8-hour shift.
