Job Title: Home-Hospice Care Operations Profession: Operations Specialty: Home-Hospice Care Operations Duration: 13 weeks Shift: 8hr Day Shift Hours per Shift: 40 Experience: Minimum of 2 years of hospice experience required License: Valid driver's license Certifications: BLS certification required Must-Have: EPIC Remote Client experience required Description: This is a home hospice position. Travelers will cover a designated area for patient care. Most patients are located in a specific city area. The position involves care for adult end-of-life patients. Candidates must possess skills in end-of-life care planning, wound assessments, care coordination, and pain/symptom management. Triage experience is also required. On-call duties will consist of one call shift per month. The schedule will include one weekend rotation each month. Any color scrubs or business casual attire is acceptable. Flexibility is necessary for weekend and on-call scheduling. All support will include RNs and case managers.
