Job Description Job Description Role and Responsibilities By Your Side Care Management is an Aging Life Care Management company looking for an experienced RN or MSW. Primary tasks: Provide assessment, home visits, and oversight in managing health care needs. Support the By Your Side team working part time during the week with on call coverage for weekends and evenings. Position includes on-call responsibilities. We serve clients in the Green Bay and surrounding area. · Medication management including filling medication box, refills, and picking up prescriptions as needed (RN/LPN only) · Communicate with client’s designated care team, family, and other contacts as indicated in care plan · Coordinate and arrange needed services based on medical, social and cognitive needs as indicated in the care plan · Refer to community resources and area professionals based on client need · Follow up to ensure client satisfaction with services provided and reassess future needs · Screen, arrange, and monitor in-home help · Assist with moving client to or from different care settings · Provide crisis intervention · Provide client and family education · Complete in person visits to ensure client safety, wellness, and adherence to medication regimen · Making necessary medical appointments and arranging transportation and/or attending appointments as needed · Identifying client entitlements and assisting with filling out applications/contacting entitlement resources · Monitor the client finances and bill paying if needed · Collect additional data by contacting relevant sources (e.g. physician(s), social support systems, and other care providers) in order to validate and expand the information obtained. · Synthesize and interpret the assessment data. · Document all intake and assessment information, care plan and interventions and progress towards goals. · Prioritize areas of concern in collaboration with client and support system in order to identify potential areas for intervention. · Identify options and resources that address the areas identified for intervention. · Discuss with the client the advantages, disadvantages, and costs of available/appropriate options and resources. · Work with client/responsible party in order to establish mutually agreed upon goals. · Develop action steps in order to achieve the agreed upon care plan goals. · Develop a timeline for implementation. · Other duties as assigned Qualifications and Education Requirements Requirements: Active, unencumbered RN or LPN license in the state of WI, RN preferred. Associates Degree in Nursing required, Bachelor’s Degree preferred. For MSW/BSW candidates, MSW preferred but BSW required. Certified Case Manager certification preferred. Preferred Skills Assessment skills, Knowledge of the healthcare system, Patient advocacy skills, Knowledge of the needs of the frail elderly, disabled and/or cognitively impaired client geriatric population. Experience in performing home visits. Experience with case management. Knowledge of community resources. Ability to assess cognitive, functional and social needs of the clients. Ability to interact positively, creatively, and Computer skills (Outlook, MSWord, Internet Explorer) Additional Notes Important: Must be willing to work weekends and nights as needed to assist with on call coverage rotation - current rotation is every 6 weeks. Must have use of an active cell phone to receive on call phone calls and respond as required by on-call policy. Must have use of a personal computer for documentation in web based documentation system. This list is not all inclusive. Care Manager may be asked to assist with other tasks as needed.
