Job Description:

Encompass Health is seeking a dedicated Pharmacist to join our healthcare team, delivering first-class pharmacy services in rehabilitation and home health/hospice settings. In this role, you will be pivotal in assisting patients to regain their independence post-serious illness or injury by providing effective medication management and patient education. This position requires a compassionate approach and a commitment to excellence in healthcare. Dispense medications according to professional standards and statutory guidelines. Monitor patient health and progress to adjust prescriptions as needed. Educate patients on drug therapies, possible side effects, and interactions. Collaborate with other healthcare professionals to plan, monitor, and review medical care. Maintain accurate records of all medications in compliance with legislation. Support patients with chronic disease management. Contribute to the selection and implementation of new pharmaceutical supplies and technology. Strong knowledge of drug administration and health care regulation. Excellent interpersonal and communication skills. Analytical skills with attention to detail. Ability to multitask in a fast-paced environment. Proficient in medical software and patient management systems. Medical Insurance Dental Insurance Vision Insurance 401k Current and unrestricted Pharmacist license. Board certification in Pharmacotherapy (BCPS) preferred. CPR certification is beneficial.
