Job Description:
Hourly Pay Range: $33.96 - $52.64 - The hourly pay rate offered
is determined by a candidate's expertise and years of experience,
among other factors. Position Highlights: - Position: RN Triage -
Transitional Care Clinic - Location: 120 Spalding Drive, Naperville
- Full Time 40 hours per week - Hours: M-F 8:00-4:30 ? no evenings
no weekends Required Travel: No What you will do: - Room patient,
record vital signs and brief history from patient. - Completes
patient admission assessment, including database and medication
list - Provides SBAR handoff for APP and pharmacist - Ability to
work as assigned direct care nurse. - Completes patient discharge
process, including After Visit Summary, and ensures medication
reconciliation completion. - Accepts other RN responsibilities as
requested by the Manager of TCC. - Reports to Manager, Transitional
Care Clinic for initial and subsequent assignments. What you will
need: - Bachelors Degree Science - Preferred - 1 Year of telephone
triage experience preferred, preferably in medical practice setting
- Preferred - Registered Nurse (RN) - Illinois Department of
Financial and Professional Regulation (IDFPR) Current State of
Illinois Registered Nurse licensure - Required - American Red Cross
CPR BLS or issued by American Heart Association - Required Benefits
