population Seeking a Board-Certified Geriatric Medicine Physician
Bi-lingual English / Spanish Outpatient responsibilities only
Full-time permanent position Monday " Friday No nights, no weekends
Small patient panels for consistency and focus 12-15 patients per
day Value-based care model and competitive bonuses are structured
to reward outcomes Provide personalized primary care for older
adults on Medicare, with the goal of keeping patients healthy and
living life to the fullest Location: Elgin, IL 35 miles NW of
Chicago along the Fox River. Known for its rich history,
particularly its connection to the Elgin National Watch Company,
and for its vibrant arts and cultural scene, including the Elgin
Symphony Orchestra and the Hemmens Cultural Center. Salary and
Benefits Above-market compensation Base salary (without R.V.U.
targets) Quarterly bonus based on qualitymetrics(not quantity)
Partnership track 6 weeks of PTO, inclusive of PTO, major holidays,
