Details - Home health setting with a territory up to a 60-mile radius from the main office - Geriatric patient population with an average caseload of 5-7 patients per day - Day shift schedule: 8:00 AM to 5:00 PM, five 8-hour shifts per week - On-call rotation required every 3rd to 4th weekend - Uses HCHB (Homecare Homebase) EMR; proficiency is required - Oasis documentation required for SOC (Start of Care) visits Requirements - Active Physical Therapy license required if applicable - Minimum 1 year of recent home health experience required - Proficient Oasis and HCHB experience required - Current BLS certification required - Flexibility with coverage area is required - Timely documentation and communication are essential Additional Information - Candidates must wear scrubs in any color; jeans are not permitted - Travelers will be provided with an iPad for documentation and mileage is reimbursed at the IRS standard rate - Strong emphasis on timely and accurate record keeping and professional communication - Applicants must be comfortable with independent work and managing daily scheduling - Local candidates are accepted at the travel rate - RTO (requested time off) must be disclosed at the time of application American Traveler Job ID P-617619.
