We are seeking a dedicated full-time Registered Nurse (RN) for our Home Health department who will uphold our mission of exceptional care. This role is crucial in providing personalized nursing care to patients in their homes, facilitating their recovery and supporting their families. Assess patient health problems and needs, and develop nursing care plans Monitor, record, and report symptoms and changes in patient conditions Administer nursing care to ill, injured, or disabled patients Advise patients on health maintenance and disease prevention Coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans Strong clinical skills with a focus on patient-centered care Ability to work independently and manage time effectively Proficiency in using healthcare software and systems Excellent communication and interpersonal skills Capacity to handle emotional and physical stress Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license in the applicable state Basic Life Support (BLS) Certification Driver’s license and reliable transportation
