How you’ll make an impact in this role - Deliver compassionate, high-quality direct patient care that aligns with established best practices and organizational protocols. - Manage and continually assess the patient care journey, meticulously documenting progress and communicating critical status updates to the multidisciplinary team to ensure continuity of care. - Coordinate care across all clinical disciplines, acting as the primary liaison to guarantee seamless and well-integrated patient services. - Execute physician and nursing orders accurately and promptly to advance patient treatment plans. - Facilitate comprehensive discharge planning by assessing patient needs and collaborating with the healthcare team to ensure a smooth and successful transition home. What minimum requirements you’ll need Licensure / Certification / Registration: - Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State. - Pediatric Advanced Life Support preferred. American Heart Association or American Red Cross accepted. Education: - Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
