Certified Occupational Therapist Assistant Location: Libertyville, IL Agency: Jackson Therapy Partners Pay: $1,023 to $1,223 per week Shift Information: Days - 5 days x 8 hours Start Date: ASAP About the Position We're hiring a Certified Occupational Therapy Assistant for a travel contract role at a Skilled Nursing Facility in Libertyville, IL. Minimum Requirements Associate degree in Occupational Therapy from an accredited college. Active Illinois COTA license required to start the assignment. BLS certification from AHA or ARC may be required. Experience working in skilled nursing facility or long-term care settings preferred. Ability to document treatment plans and progress notes per facility and state regulations. Experience with ADL retraining, adaptive equipment recommendations, and fall prevention programs preferred. Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days. Shift Details: Days 08:30–16:30. Schedule: Five 8-hour shifts per week.
